Angst over carrying SA pistols (ie. 1911, Hi Power, etc.) cocked and locked can be a concern with some people; fearing the lever will fall out of place. Does anyone know first hand of such an incident taking place?

    Yes.

While I don't think it can just "fall out of place", it can be accidentally switched to "safety off". If the safety lever does not stay in place, if it is loose, you may have a problem.

I carry a Sig P238 or a High Power equipped with SFS. Every so often, actually not that often at all, I discover the safety is off when I put it away for the day. This has never happened with my SFS High Power.

Here is a link to a BHSS page offering SFS for sale. It is available only for High Powers and 1911s. I dearly wish it was available for the Sig P238. I find it to be an elegant upgrade to the 1911 style SA cocked and locked platform. I have been using a High Power with SFS since 2002 and I am a fan.
